Acute effects

Acute effects results from exposure to high level of substance over a very short period of time, typically effects quickly after the exposure.  Eg. itching, skin irritation, cold etc
or 
Short-term exposure with immediate effect, usually reversible

Chronic effects

Chronic effects resulted from exposure to lower levels of the substance over long period of time usually weeks, months or years after the exposure. Eg. cancer, aids etc
or
Long-term or repeated exposure with delayed effect, often irreversible
